BIRTH CERTIFICATE Smith apparently filled out his own birth certificate on June 6, 1921, probably to apply for a passport the same month.

1900 CENSUS Smith was living with his father, George in New York City, and employed as a musician.

1910 CENSUS Smith was now married and living in Brooklyn, also employed as a musician.

1915 NEW YORK CENSUS Smith was living at the same address as he was in the 1910 census.

SUPPLEMENT NOTES A collection of notes from monthly record supplements.

SHEET MUSIC Smith and a guest musician, trombonist Harry Raderman, appear on the front cover of "Yellow Dog Blues" from 1919. The lyrics are included.

1920 CENSUS Smith was living at the same address as he was in the 1915 New York census.

DRAMATIC MIRROR A full page advertisement for Smith at the Plaza Hotel in New York City from June 1920.

MARGARET LYNCH'S PASSPORT APPLICATION Joseph C. Smith signed the passport acknowledging that he knew Margaret Lynch since 1915. She would later become Smith's wife.

PASSPORT APPLICATION Smith applies for a passport to travel abroad on June 30, 1921.

PASSENGER LIST Smith returns home from abroad on August 9, 1921.

HUGO FREY AND HIS STRING ORCHESTRA Hugo Frey formerly worked as Smith's pianist and technical director, as this 1925 Variety review proves.

SMITH BACK AT PLAZA? Rumor had it that Smith's orchestra was returning to the Plaza Hotel in New York City in 1925, but the orchestra directory in Variety still listed Smith at the Mount Royal Hotel in Montreal, Canada later that year.

MUSICIAN OF FAME BUYS CAR A 1927 Los Angeles Times article about Smith purchasing a new Chrysler.

SMITH'S ORCHESTRA IS WALES FAVORITE Smith goes back into radio in New York City in early 1930. The article also mentions that his orchestra was a favorite of the Prince of Wales.

1930 CENSUS Smith is living in New York City, employed as a musician.

SOCIAL SECURITY APPLICATION Dated January 9, 1937.

DEATH CERTIFICATE Died Monday, March 22, 1965.
